Our Active Living – Exercise Referral scheme is a great way for people to increase physical activity levels and improve their health.
The scheme covers a wide range of health conditions including…
High blood pressure, diabetes, asthma, arthritis, overweight or obesity, depression, anxiety, osteoporosis and more – all of which we can help you with.
If you would like to positively influence your health and wellbeing our trained team are ready to get you started with a variety of enjoyable physical activities.
We run the Active Living referral scheme at the following centres:-
Bury | Brandon | East Bergholt | Holbrook | Hadleigh | Haverhill | Newmarket | Sudbury (see the timetables of activities below)
To get started, simply download the ‘referral form’ below, ask your GP to complete it and bring it along to your first session.

AWARDED THE SUFFOLK EXERCISE ON REFERRAL QUALITY STANDARD
Active Living is the first referral scheme to be awarded the Suffolk Exercise on Referral Quality Standard.
The new quality standard was developed by Suffolk County Council Public Health, and county wide partners, for Exercise Referral Schemes (ERS) operating within the county.
You will be able to enjoy exercise in a supported environment with qualified instructors on hand to provide advice, guidance and personalised exercise plans. Our specialist instructors will help you work towards your personal goals.
